Your fellowship program voluntarily participates in the AUPO FCC compliance process. By doing so, the program has agreed to meet specific requirements to maintain its compliant status. The Fellow Exit Survey and Procedure Log is an important piece in the process and is required in order to assess the program's continued compliant status. It is highly recommended that you maintain your Surgical Log throughout your fellowship.
In an effort to help maintain your contact information, Procedures Log and the completion of the Exit Survey as easy as possible, you may login to your account at any time throughout your fellowship.
The AUPO FCC has created an account for you and login details were emailed to you when your program verified your information.
At anytime you may review your subspecialty’s requirements under the Subspecialty section of the website. This document explains the requirements for education in a fellowship for your program and provides guidance about the information you will need to track to successfully complete the procedure log and exit survey.
Your Fellow Exit Survey must be completed no later than June 30th. If your fellowship will end after June 30th please email aupofcc@aao.org to request an extended deadline.
